Vision: provider needs
- Tell your provider that you have VisionPlus powered by VSP™ Vision Care.
- An ID Card is not necessary, but you can access your ID Card on vsp.com. Without an ID Card, the provider will need the patient’s name, date of birth, and the last 4 digits of the Primary Insured’s Social Security number.
